Signify Health acquires TAVHealth to address social determinants of health

Signify Health’s mission to improve quality of life  

Signify Health, a health-tech company that helps monitor patients outside of the hospital has acquired TAVHealth, a software-company that provides healthcare services, to boost its services in Social Determinants of Health (SDOH). The acquisition of TAVHealth provides Signify a path to address social determinants of health. Furthermore, it focuses on needs outside of clinical care such as food insecurity, transportation issues, and unstable housing within its internal system.

Checking the patients environment at home

Signify Health tracks patients with chronic conditions by sending over a care team to check the patient’s environment at home like the availability of fresh food in the fridge, prevention of accidental falls and whether caretakers are present to look after them or not.

Kyle Armbrester, CEO of Signify Health, stated “A surprisingly high percentage of health outcomes are driven by social, environmental, and behavioural factors. Signify Health makes a record of a patient’s health outcomes based on the visit of the care team.”

Working together to form organized social health records and improve health services

Signify Health will improve its service by including TAV Health’s expertise in building organized social health records. With the help of TAVHealth’s technology and privacy capabilities cross-sector care teams can collaborate, coordinate services, keep track of patients and create longitudinal social records together. TAVHealth’s tech-enabled service solution provides a one-of-a-kind privacy architecture required for true collaboration between community and healthcare partners. Additionally, this privacy-enabled collaboration gives care teams the ability to build a longitudinal record which is a live timeline of all evaluations performed, needs identified, referrals shared, and services provided over time. Besides that, it prevents unaddressed social risks from flowing into the health system and improves health, quality, and financial outcomes.
